Informed Consent Form

Dear Parent,

We invite you to join a study aimed at helping parents better understand and monitor their young children's breathing. Respiratory distress in small children can be a cause for concern, and we are working to support the creation of an application that utilizes smartphone cameras to assess whether a child may be experiencing respiratory distress.

Our goal is to build a database of breathing movements in both healthy and sick children. This database will be analyzed with artificial intelligence to support the development of an application that can accurately identify respiratory distress, improving the care and management of children worldwide facing breathing issues.

Your participation is crucial. We kindly ask for your help in recording a short video (20-30 seconds) of your child's chest and abdominal movements using your smartphone. You can do this at your convenience, and the video should not include any identifying information about your child.

After signing this informed consent, you will be asked to answer general questions about your child (e.g., age and gender) and upload the video to a secure website owned by the Ministry of Health. Any identifying details will be carefully removed during the video review before it is entered into the database.

Your data will remain anonymous, including your contact details, ensuring your privacy. Your contribution will be handled with utmost care, and the research results will not compromise your or your child's privacy.

If, at any time, you no longer want your child's study information or video to be used in this research, you can request your child's data to be withdrawn and destroyed.

The study is led by Prof. Israel Amirav from the Children's Pulmonary Unit at Ichilov Hospital in Tel Aviv, in collaboration with children's hospitals in Toronto, Canada, and the Congo.
